The woman in the blue dress holds a small boy doll that has been with her since childhood. He doesn't seem to have childlike facial features and thus either has portrait and adult from while the girl - was little - analog to the Ken dolls or the "action figures" of the pre-robot times. Or that doll has been sent out the "service" with the intention to parallel his age with the coming of age of the girl.
The girl didn't get bored of him, she still feels a strong emotional attachment to him. She loves him despite his unrealistic size and looks.
The other figures of the painting, actually even more so in case for the later versions, give wide room for interpretation.

Title: "Very mysterious doll"
With a bit of irony, this version speaks the mind of the typical recipient of a contemporary (or modern or often even classic) art work who hasn't yet read about the artwork he/she is viewing . What might mean what?
The handwriting is so far from good readability that it well symbolizes the insecurity even of formulating the questions one would like to ask the work or its artist.

Lessing has chosen a rather sexualized context for this version of the Tiny Man. Three women have their tongues stick out, the second from the right is presenting her behind to the viewer, the central woman on the left has her eyes rolled up and drools heavily.
Well, it's again up to the viewer to imagine the narrative of the scene.
